Job summary

CenterWell Home Health seeks a dedicated Physical Therapist to help patients regain strength, mobility and independence in the comfort of their homes, reporting to the Clinical Manager and collaborating with the care team.

You will plan and administer prescribed therapy, assess strengths, develop rehab plans, supervise PTAs and Home Health Aides, document progress, and coordinate with physicians and families to support functional goals.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• One year of experience as a Physical Therapist (preferred).

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients in their homes.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop and implement rehabilitation programs aligned with physician plans and patient goals.
• Provide supervision and written instructions to PTAs and Home Health Aides.
• Document patient observations, interventions, and progress.
• Report patient progress to physicians, Clinical Manager, care team, patient and family.
