Job summary

Qualifications

• Associate degree in Physical Therapy Assistant (PTA) as required.
• BLS CPR certification required.
• Driver's License required.

Responsibilities

• Coordinate with case manager, primary therapist, and other healthcare professionals to facilitate interdisciplinary continuity of care and discharge planning.
• Maintain productivity standards and adjust work hours to meet patient needs.
• Document interventions and patient progress accurately and timely per standards of practice.
