Job summary

AdventHealth in Altamonte Springs, FL, seeks a licensed Physical Therapist to develop and reassess patient-specific home health care plans, coordinating with families and physicians to meet physical therapy goals.

This full-time day-shift role requires a Doctorate in Physical Therapy preferred, current PT license, CPR/BLS, and the ability to work in a Medicare-certified home health setting.

Qualifications

• Doctorate in Physical Therapy preferred.
• Field of Study: Physical Therapy.
• 1+ year clinical physical therapy experience required.
• Medicare-certified home health experience preferred.
• PT license and CPR/BLS required.

Responsibilities

• Develops patient-specific care plans with goals with patients, families, and physicians.
• Reassesses patients, documents progress, and adjusts therapy.
• Documents treatments in medical records and updates plans.
• Coordinates and directs care for home patients under PT.
• Prioritizes and adapts home care caseloads to meet needs.
• Performs evaluations of functional abilities and documents results.
• Selects and modifies interventions to achieve best outcomes.
• Maintains compliance with agency policies and contributes to staff education.
