Job summary

AuthoraCare Collective is seeking a Physical Therapist Assistant (PTA) Case Manager for Home Health. This full-time role runs Monday through Friday, 8:00am to 5:00pm, providing services to restore function and improve mobility under the supervision of a Physical Therapist.

The PTA will document goals and outcomes, work with an interdisciplinary team, and maintain treatment plans for home health patients, carrying out various therapeutic methods and equipment usage to ease pain and increase

Qualifications

• CPR certification required.
• Experience collaborating with interdisciplinary teams (IDT).
• Documentation of patient goals and outcomes.

Responsibilities

• Directs and coordinates patient care per Plan of Treatment.
• Assess patient/family needs and adjust Plan of Care.
• Maintain therapeutic relationship and provide education to patient/caregiver.
• Identify patient/family goals with IDT and implement Plan of Care.
• Communicate Plan of Care changes to patient/family/facility.
• Follow infection control policies.
