Job summary

AuthoraCare Collective in Greensboro, NC is seeking a Physical Therapist (PT) Case Manager for our Home Health department to support Greensboro, Kernersville and Winston-Salem service areas. This is a full-time position, Monday through Friday 8:00am to 5:00pm.

The PT will manage the therapy Plan of Treatment for Integrated Health patients, assess problems, set goals, and collaborate with the interdisciplinary team to restore function and mobility using a variety of methods.

Qualifications

• Graduate degree from an accredited Physical Therapy program (APTA approved).
• Minimum of one-year physical therapy experience; home health or facility preferred.
• Current and unrestricted Physical Therapy licensure.
• Current CPR certification.
• Patient management skills include assessment, critical thinking, problem solving and communication.

Responsibilities

• Manage the therapy plan of treatment for a case load of Integrated Health patients with IDT collaboration.
• Assess patient problems, establish therapy goals, and implement interventions.
• Develop treatment plans using exercises, manual therapy and equipment to improve mobility.
• Maintain therapeutic relationships and document goals and outcomes of care.
