Job summary

CenterWell Home Health is seeking a qualified Home Health Physical Therapist Assistant who reports to the Clinical Manager. You will administer the physical therapy treatment plan to patients in their homes, focusing on functional improvement, mobility and independence to help clients regain strength and confidence.

Responsibilities include documenting records, coordinating with the Physical Therapist and Branch Director, attending department in-services, and maintaining infection control

Qualifications

• Associate degree in Physical Therapy from an American Physical Therapy Association.
• Current state license to practice Physical Therapy.
• Valid driver’s license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric physical, emotional, social, and biological changes.
• Read, write, and speak fluent English.
• One year of experience as a Physical Therapy Assistant (preferred).

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supply shortages, or staff or client changes.
• Apply infection control and universal precautions in performing patient care activities.
