Residential Home Health and Hospice is seeking a dedicated Physical Therapist Assistant to deliver skilled PT services in patients’ homes across Marion, Carbondale, and surrounding areas. You’ll work under a licensed PT guiding mobility, pain reduction, and independence through targeted therapies.

This role requires an active PTA license in the state, at least one year of PTA experience, and a valid driving license for approximately 20% travel.

Qualifications

• Active PTA license in the state you work.
• One year of experience as a Physical Therapist Assistant and the clinical expertise to deliver quality patient care.
• A valid driver’s license with ability to drive ~20% of workday to patient locations.

Responsabilités

• Provide compassionate, patient-centered Physical Therapy services in the home health setting under the direction of a licensed Physical Therapist.
• Support patients in restoring mobility, reducing pain, and maximizing functional independence through therapeutic exercises, gait training, transfer training, muscle re-education, and prosthetic or orthotic support.
• Educate patients and caregivers on the safe use of mobility and assistive devices, while monitoring progress and communicating patient responses and condition changes to the care team.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
