Frontier Home Health is seeking a compassionate Physical Therapist Assistant (PTA) to provide skilled services in patients' homes under a supervising Physical Therapist. You will help improve mobility and overall quality of life through individualized care plans.

We offer full benefits and flexible schedules with opportunities for ongoing education. A Colorado PTA license, CPR, and a valid driver’s license are required to join our team.

Qualifikationen

• Compassionate patient care for home health settings.
• Ability to plan and adjust therapy programs.
• Accurate documentation and charting in EMR.

Aufgaben

• Deliver PTA services under PT supervision in patients' homes.
• Assist with mobility training, therapeutic exercises, and transfers.
• Use modalities as prescribed and monitor patient responses.
• Document treatment plans, progress, and changes in EMR.
• Ensure compliance with Medicare/Medicaid guidelines and HIPAA.
