Responsibilities

• Provide compassionate, patient-centered PTA services in home health under the direction of a licensed PT.
• Assist patients to restore mobility, reduce pain, and maximize independence through therapeutic exercises and training.
• Educate patients and caregivers on safe mobility and device use; report progress to the care team.
• Maintain thorough, compliant EMR documentation and ensure patient progress is tracked.
• Travel to patient homes within the assigned territory.
