Job summary

Qualifications

• Associate degree in physical therapy from an accredited college or technical school required.
• Minimum one year of experience in an acute care and/or rehabilitative setting preferred; home health experience preferred.
• Must hold, in good standing, a license/registration as a Physical Therapist Assistant.

Responsibilities

• Deliver and manage patient care plans in collaboration with the care team.
• Support Springfield-area patients with home health PT services and charting.
• Demonstrate commitment to timely, quality patient care and safety.
