UnitedHealth Group is seeking a Physical Therapist Assistant to provide skilled in-home care under a licensed Physical Therapist. You will implement treatment plans to improve mobility and safety, support recovery, and enhance quality of life for patients in their homes.

Responsibilities include documentation, collaboration with the care team, and participation in quality improvement activities. A PTA license, CAPTE accreditation, CPR, and 1+ years of home health experience are required.

Qualifications

• Graduate from an accredited PTA educational program.
• Current license to practice as a PTA in the assigned state.
• CAPTE accreditation.
• Current CPR certification.
• 1+ years of PTA experience in home health care or equivalent.

Responsabilités

• Provides physical therapy services under the supervision of a licensed PT and according to the plan of care.
• Observes and reports the patient’s response to treatment and any changes to the supervising PT.
• Provides resources for future needs and promotes wellness and prevention.
• Documents supervisory activities in the patient’s medical record.
• Monitors and documents patient progress and productivity targets.
• Participates in care conferences and quality improvement activities.
• Maintains confidentiality of patient information.
• Arranges follow-up care with relevant agencies and resources.
• Performs other duties as assigned.
