Job summary

VitalCaring Group is seeking a dedicated Home Health Physical Therapist to deliver patient-centered care in home settings. You will evaluate mobility, design individualized plans with physicians, and provide hands-on therapy to improve independence and safety.

This role offers autonomy in scheduling, collaboration with nurses and therapists, and opportunities for ongoing clinical education and career advancement. Valid PT licensure and CPR are required.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Document patient care accurately in the EMR and participate in team conferences.
