Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to evaluate and treat patients in the home, using physical modalities to improve mobility and daily living skills. You will work under the physician's orders and the plan of care, with a focus on maximizing independence and safety.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities to improve mobility and daily living skills.
• Administer skilled care under physician orders and the established plan of care, and educate family and health care team members.
