Job summary

Boost Home Healthcare is seeking a Licensed Physical Therapist to provide physician-prescribed PT to patients in their homes in Massachusetts. You will assess, plan and implement care to improve functional abilities and help patients return to their highest level of independence.

Ideal candidates hold a PT degree and current MA license, with preference for those having at least two years of home health experience and familiarity with Medicare OASIS documentation.

Qualifications

• Graduate with a Bachelor’s, Master’s, or Doctorate in physical therapy from an approved program.
• Currently licensed in the state in which practicing.
• Two years’ experience preferred in a home health setting.
• Knowledgeable and experienced with Medicare OASIS documentation (optional).

Responsibilities

• Provide physician prescribed physical therapy.
• Improve or minimize residual physical disabilities of the patient.
• Return the individual to optimum and productive level within the patient’s capability.
• Participate with all other home care personnel in patient care planning.
• Direct and supervise personnel as required.
• Responsible for initial assessment, plan of care, maintenance program development and modifications and reassessments every 30 days.
