Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is hiring a Home Health Physical Therapist to serve Dublin, GA. You will provide 1:1 patient-centered care in home settings, perform evaluations, develop treatment plans, and help patients regain independence.

We offer flexible schedules (FT/PT/PRN), sign-on up to $15,000 and relocation assistance for external candidates, competitive benefits including medical, dental, vision, 401(k), mileage reimbursement, and tuition reimbursement.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Active license in the state of practice.
• Current CPR certification.
• Two years of physical therapy experience recommended; hospice/community experience preferred.

Responsibilities

• Perform physical therapy evaluation and treatment per physician orders, including adaptive equipment as needed.
• Establish measurable goals and develop treatment plans to maximize independence and function.
• Maintain communication with the treatment team, physician, patient, and family as applicable.
• Maintain thorough and timely documentation for proper billing.
• Supervise PT Assistants and PT Aides in patient care per state practice act.
