Adoration Health is seeking a Home Health Physical Therapist for Dublin, GA. Provide 1:1 patient care in home settings, evaluating and treating patients per physician orders with a focus on independence and quality of life.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• New graduates may be considered in select markets.
• At least 2 years of PT experience; hospice/community experience preferred.
• Valid license in state of practice; CPR certification required.

Responsabilités

• Perform PT evaluation and treatment per physician orders, including adaptive equipment.
• Set measurable goals and develop treatment plans to maximize independence.
• Maintain communication among treatment team, physician, patient and family.
• Maintain thorough documentation for proper billing.
• Supervise PTAs and PT aides in direct patient care.
