Job summary

Qualifications

• Bachelor's or Master’s in Physical Therapy from an accredited college.
• Licensed Physical Therapist issued by the state where you will work.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• Commitment to consistently meet critical deadlines for charting.
• Skills to self-manage time and scheduling.
• Experience with tablets and mobile devices and EMR software.

Responsibilities

• Manage and deliver patient care plans in home health.
• Collaborate with the care team to coordinate services.
• Maintain accurate documentation and charting for patient visits.
