Job summary

Graham Healthcare Group, through Residential Home Health and Hospice, seeks a Home Health Physical Therapist to deliver patient-centered care in North Chicago, Illinois. This full-time staff role requires 40 hours per week with an 8-hour shift and travel to patient locations as needed.

Must have a Bachelor's or Master's in Physical Therapy, a state license, and at least one year of healthcare experience. A valid driver's license and reliable transportation are required.

Qualifications

• Bachelor's or Master's degree in Physical Therapy required
• Licensed Physical Therapist in the state where you will work
• Minimum of one year healthcare experience
• Valid driver's license and ability to travel ~20% of time
• Commitment to timely charting and self-management of schedule

• Provide management and delivery of patient care plans in home health setting
• Collaborate with care team to ensure high-quality patient outcomes
• Maintain accurate EMR documentation and coordinate with schedulers
