Job summary

Mid-Michigan Home Health and Hospice is hiring a Physical Therapist Assistant to deliver skilled PT services in patients' homes under the supervision of a licensed PT. You will guide exercises, mobility training, and safety measures to improve independence and daily functioning.

You will educate families, monitor progress, and ensure compliance with Medicare/state requirements while maintaining compassionate, patient-centered care in a flexible schedule within the Mid-Michigan region.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Michigan Physical Therapist Assistant license.
• Knowledge of therapeutic exercise, gait training, transfer techniques, and rehabilitation principles.
• Strong communication, organizational, and time-management skills.
• Ability to work independently and manage a flexible daily schedule.
• Valid driver's license and reliable transportation.
• Compassionate and professional approach to patient care.

Responsibilities

• Provide skilled physical therapy treatments in the patient's home in accordance with the established plan of care.
• Assist patients with therapeutic exercises, strengthening, balance training, gait training, and functional mobility.
• Implement interventions designed to improve safety, mobility, endurance, and independence with activities of daily living.
• Educate patients and caregivers on prescribed exercises, safe transfer techniques, fall prevention, and use of assistive devices.
• Monitor patient responses to treatment and report changes in condition, functional status, or safety concerns to the supervising Physical Therapist.
• Document patient progress, treatment interventions, and responses to therapy accurately and timely.
• Communicate effectively with Physical Therapists, nursing staff, physicians, and other members of the interdisciplinary team.
• Assist with assessing patient progress toward established therapy goals and communicate when modifications to the plan of care may be needed.
• Maintain compliance with Medicare regulations, state practice requirements, and agency policies.
• Demonstrate professional, compassionate, and patient-centered care during every visit.
