Job summary

PARS Therapy in Liberty, TX is seeking a compassionate Physical Therapist to join our home health team. You will evaluate patients in their homes, develop individualized plans, and deliver evidence-based interventions to improve mobility and reduce pain.

You will educate patients and caregivers, document progress, and collaborate with an interdisciplinary team to ensure safe, effective care. Strong communication skills and TX licensure are essential.

Qualifications

• Active Texas Physical Therapist license.
• Professional liability insurance coverage for TX.
• Graduate degree or higher in Physical Therapy.
• Home health or rehabilitative experience preferred.
• Strong communication and patient education skills.

Responsibilities

• Evaluate patients’ physical abilities in home health.
• Develop and implement individualized PT plans.
• Provide therapeutic interventions: exercise, gait, manual therapy, balance retraining.
• Educate patients and caregivers on home exercise programs.
• Monitor progress and adjust plans for optimal outcomes.
• Document evaluations, sessions, and progress digitally.
• Collaborate with physicians, nurses, and occupational therapists.
• Conduct home safety assessments and fall-prevention recommendations.
• Deliver compassionate care and empower patient independence.
