Job summary

Millennium Physician Group is seeking a Physical Therapist to join our Home Health Team in Lee County. This PRN position involves evaluating patients in their homes, developing treatment plans, and providing interventions to improve mobility and independence.

You will educate families, document care, and collaborate with a multi-disciplinary team to ensure quality outcomes. The role requires a current Florida PT license and at least 1 year of clinical PT experience, preferably in home health.

Qualifications

• Licensure: Current and valid Physical Therapist license in Florida.
• Experience: Minimum 1 year of clinical PT work, preferably home health.
• Professionalism: Strong work ethic and patient-centered approach.

Responsibilities

• Assess patients' physical abilities and develop individualized plans.
• Perform therapeutic interventions including manual therapy and exercises.
• Educate patients and families on exercises, body mechanics, and recovery.
• Collaborate with interdisciplinary team to optimize outcomes.
• Maintain accurate documentation of evaluations, treatments, and progress.
• Comply with HIPAA and Medicare regulations.
