Job summary

CenterWell Home Health is seeking a licensed Physical Therapist to provide in-home care, helping patients regain strength, mobility, and independence. You will assess, plan, and implement rehabilitative programs, while coordinating with PTAs, home health aides, and physicians to optimize outcomes.

You will document progress, communicate with the care team and family, and ensure timely submission of all evaluations and discharge summaries.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain the highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with the physician's Plan of Treatment and the goals of the patient team.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
