Job summary

PARS Therapy is seeking a compassionate and skilled Physical Therapist (PT) to join our home health team in Cotulla, TX. This onsite role focuses on evaluating patients, creating personalized treatment plans, and delivering evidence-based care in patients' homes.

The ideal candidate will hold a TX PT license, possess a graduate degree in Physical Therapy, and have experience in home health or rehabilitative settings.

Qualifications

• Active Physical Therapist license in TX.
• Professional liability insurance coverage.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to diverse patient needs.
• Up-to-date knowledge of evidence-based treatment techniques and best practices.
• Excellent communication and patient-education skills.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies.
• Monitor patient progress and adjust plans to achieve optimal outcomes.
• Accurately document evaluations, treatment sessions, and progress notes using digital platforms.
• Collaborate with physicians, nurses, and other therapists as part of an interdisciplinary team.
• Conduct home safety assessments and make recommendations to prevent falls or re-injury.
• Deliver compassionate care while empowering patients to achieve greater independence.
