Job summary

Enhabit Home Health & Hospice in Massachusetts is seeking a Licensed Physical Therapist to provide skilled PT during home visits, utilizing modalities to enhance mobility and independence for patients in their own homes.

You will collaborate with a clinical team, participate in Balance and Fall Prevention programs, and contribute to individualized care plans under physician orders, ensuring safety and quality in a growing home-based care network.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments in the home using physical modalities.
• Aim to improve mobility and independence in activities of daily living.
• Contribute to specialty programs such as Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement, and Total Shoulder Replacement.
• Administer skilled care under physician orders and established plans of care.
• Educate families and health care team members to support ongoing recovery.
