Job summary

AdvisaCare in Michigan is seeking Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to deliver one-on-one care in patients’ homes, guiding rehabilitation and personalized treatment plans.

As part of an interdisciplinary team, you will perform evaluations, implement plans, and educate families on safe exercises, transfers, and ambulation. This role emphasizes compassionate service, flexible scheduling, and a rewarding work environment that strengthens communities.

Qualifications

• Degree from an accredited Physical Therapy or Physical Therapist Assistant program.
• Current licensure as a Physical Therapist or Physical Therapist Assistant in Michigan.
• CPR certification.
• Strong interpersonal and communication skills; able to work independently and in a team.

Responsibilities

• Assist in implementing individualized treatment plans developed by the licensed Physical Therapist.
• Conduct therapeutic exercises and activities to improve mobility, strength, and coordination.
• Provide guidance and education to clients and families on exercises, transfers, and ambulation.
• Assess and document clients' progress and adjust treatment plans as needed.
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care.
• Communicate effectively with clients, families, and caregivers, offering support and answering questions.
• Adhere to all relevant legal, ethical, and professional standards.
