Job summary

AdvisaCare seeks licensed Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to deliver one-on-one home-based rehabilitation in Michigan, including Cadillac. You will assess mobility, set goals, and create personalized plans to restore function and independence.

PTs conduct comprehensive evaluations and oversee therapy services; PTAs provide hands-on treatment under PT supervision. Join a patient-centered, interdisciplinary team focused on compassionate care.

Qualifications

• Degree from an accredited Physical Therapy program and an approved PTA program.
• Current state licensure as a Physical Therapist and/or Physical Therapist Assistant (Michigan).
• CPR certification.
• Strong interpersonal and communication abilities are essential.

Responsibilities

• Assist in implementing individualized treatment plans developed by the PT.
• Conduct therapeutic exercises to improve mobility, strength, and coordination.
• Educate clients and families on safe exercise techniques and transfers.
• Assess and document progress; adjust treatment plans as needed.
• Collaborate with interdisciplinary care teams to ensure continuity of care.
• Communicate with clients, families, and caregivers effectively.
• Adhere to legal, ethical, and professional standards.
