Job summary

Live Well Homecare Agency, LLC is seeking a licensed Physical Therapist to provide direct patient care in home health settings. Responsibilities include assessment, treatment planning, and supervision of PT practices as ordered by physicians.

Role requires instructing patients and families, assisting with activities and device use, and re-evaluating progress to optimize outcomes. Travel to community-based service areas is involved, with a patient-centered, multidisciplinary approach to care.

Qualifications

• Bachelor’s Degree in Physical Therapy (PT) required.
• Valid Driver’s License and Physical Therapist License.
• No prior experience required.
• Strong communication skills for patient instruction and collaboration.
• Ability to evaluate and adapt treatment plans.
• Proficiency in fitting and adjusting prosthetic and orthotic devices.

Responsibilities

• Perform assessments and develop treatment plans.
• Instruct and assist patients with physical activities and device use.
• Re-evaluate and modify treatment approaches for maximum benefit.
• Collaborate with healthcare team to coordinate patient care.
• Evaluate and recommend modifications for prosthetic and orthotic devices.
