Job summary

LiveWell Homecare Agency seeks a licensed Physical Therapist to provide primary PT care to home health patients across multiple counties. You will supervise therapy staff and coordinate interdisciplinary services to ensure optimal patient outcomes.

You will conduct assessments, develop care plans, and document care per regulatory guidelines while educating patients and caregivers to promote independence.

Qualifications

• Graduate of an accredited PT program.
• Valid Georgia PT license; BLS certification; Georgia driver's license with insurance.

Responsibilities

• Conduct assessments, develop and revise individualized care plans, manage patient progress, and perform routine and complex therapy procedures in patients’ homes.
• Supervise PTAs and Home Health Aides, ensuring quality care delivery throughout the episode of care.
• Provide patient and caregiver education to promote independence and self-care.
• Communicate effectively with physicians, team members, and patients, and document all care accurately following regulatory guidelines.
• Participate in performance improvement initiatives, staff training, and meet productivity standards.
