Job summary

Empower Home Health, a LiveWell Partners Network Agency, is seeking a Physical Therapist (PT) to evaluate patients in home health settings and develop treatment plans. The role includes directing PT assistants, coordinating with the care team, and ensuring effective discharge planning.

A valid license, CPR, and a driver’s license are required. The position offers a full-time schedule, field-based visits, mileage reimbursement, and a comprehensive benefits package.

Qualifications

• Active state licensure in physical therapy is required.
• CPR certification must be current.
• Valid driver's license and auto insurance coverage.
• Minimum 2 years of physical therapy experience; preference for acute or post-acute settings.

Responsibilities

• Develop and implement PT treatment plans in patients' homes.
• Document care plans and clinical notes.
• Direct and supervise physical therapy assistants.
• Evaluate progress and adjust therapy as needed.
• Coordinate discharge planning and follow-up care.
• Participate in weekly interdisciplinary case conferences.
