Live Well Homecare Agency, LLC seeks a Physical Therapist (PT) to assess, teach, and deliver therapy in patients' homes, guided by physician-directed home health plans.

You will perform initial and ongoing mobility assessments, develop and modify treatment plans, document progress, and coordinate discharge with patients and caregivers. The PT will supervise staff where permitted and ensure compliance with policies and regulations.

Qualifications

• Must hold a Doctor of Physical Therapy degree from an accredited program.
• Current unrestricted state license to practice physical therapy.
• CPR certification is required.
• Valid driver's license with proof of insurance.

Responsabilités

• Perform initial and ongoing mobility assessments.
• Develop and modify individualized treatment plans.
• Deliver therapy interventions in patients' homes.
• Document progress and communicate with care teams.
• Coordinate discharge planning with patients and caregivers.
• Supervise PTAs and HHAs where permitted, ensuring policy compliance.
