Job summary

BAYADA Home Health Care in Danvers, MA is seeking a dedicated Home Health Physical Therapist to provide skilled PT services during home visits. This per-visit opportunity offers flexible hours with ongoing cases across North Boston territories, including Saugus, Lynn, and Salem.

You will assess mobility, balance, gait, and safety, develop individualized care plans, educate clients and families, and document care in our EMR. Requires MA license and at least one year of PT experience.

Qualifications

• Current MA Physical Therapist license.
• Minimum of one year of recent clinical PT experience.
• Graduation from an APTA-approved program.
• Ability to work independently with strong organizational and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Conduct mobility, strength, balance, and safety assessments.
• Develop and adjust individualized care plans with manual therapy and gait training.
• Educate clients and families on rehabilitative care.
• Document observations and interventions accurately in the EMR.
