Job summary

Qualifications

• Current MA Physical Therapist license is required.
• Minimum one year of recent PT experience as a licensed therapist.
• Graduation from an APTA-approved program with transcript/diploma.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, strength, balance, and safety.
• Implement wound care interventions and promote wound healing with evidence-based dressings.
• Develop and implement individualized care plans with PT interventions.
• Continuously assess and revise the PT plan in coordination with nursing.
• Educate clients and families on rehabilitative care to promote independence.
• Document observations and interventions in the EMR with accuracy.
