Job summary

BAYADA Home Health Care in Swampscott, MA is seeking a licensed Physical Therapist to join our home health visits team. This part-time or full-time role covers designated territories north of Boston, providing 1:1 rehabilitative care to adult and geriatric clients in their homes.

One year of clinical PT experience and MA licensure are required. Duties include home visits, mobility assessments, developing treatment plans, documentation, and coordinating with a multidisciplinary team.

Qualifications

• Active MA Physical Therapist license required.
• Minimum of one year of recent PT experience.
• Graduation from CAPTE-accredited PT program or equivalent.

Responsibilities

• Make home visits to clients in designated territories.
• Perform mobility and functional assessments and document results.
• Develop and implement individualized PT plans including exercises and gait training.
• Educate clients and families on rehabilitative care and safety.
• Document observations and interventions in EMR per agency standards.
