Job summary

BAYADA Home Health Care is seeking an experienced Physical Therapist for home health visits in the North Boston area, including Saugus, Lynn, Salem, and Beverly. The role offers part-time or full-time schedules, ongoing case coverage, and a patient-focused, 1:1 care model in adult and geriatric home settings.

Qualified candidates will hold MA PT licensure with at least one year of PT experience, ability to work independently, strong communication skills, and EMR proficiency.

Qualifications

• MA Physical Therapist license (PT)
• Minimum 1 year of recent PT experience
• Graduation from an APTA or AMA-accredited program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills
• Solid computer skills; EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform mobility, range of motion, gait, balance and strength assessments
• Implement wound care and protective interventions with evidence-based dressings
• Develop and implement individualized care plans including exercises and gait training
• Collaborate with nursing on multidisciplinary care plans
• Educate clients and families on rehabilitative care and safety
• Document observations and interventions using a tablet EMR system
