Job summary

Premier Home Health Providers Inc. is seeking a dedicated Physical Therapist for a per diem role in California. You will evaluate patients, develop individualized plans, and deliver skilled therapy in the home setting, guiding recovery and independence with families involved.

Requirements include a PT degree, active CA license, and at least 2 years of PT experience; home health experience is a plus. A flexible schedule and meaningful impact on patient lives are offered.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.

Responsibilities

• Evaluate patients’ functional status, mobility, strength, gait, and ROM for plans.
• Deliver PT services per physician plan and California law.
• Educate patients and families on exercises, equipment use, and goals.
• Collaborate with care team and report changes in condition.
• Supervise PT Assistants and oversee Home Health Aide care bi-weekly.
• Maintain documentation including progress summaries and discharge plans.
• Identify needs for additional home health services and coordinate referrals.
• Perform other duties as required for patient services.
