Qualifications

• Active RN license in the state you work
• One year of RN experience
• Valid driver’s license and ability to drive 20% of workday
• Field-based home health experience preferred

Responsibilities

• Deliver skilled, patient-centered care in a home health setting
• Perform patient assessments, medication management, and care planning
• Educate patients and families on disease management and treatment plans
• Think critically and ensure compliant EMR documentation
• Work independently, but collaborate with interdisciplinary team
• Travel to patient homes within assigned territory
