Job summary

Compassus is seeking a Home Health Physical Therapist to work one-on-one with patients in their homes, helping them regain strength and independence through skilled therapy. You will assess, plan, and implement individualized treatment while collaborating with families and the care team.

Qualifications

• BS in Physical Therapy and current state PT license.
• CPR current; TB screen and Hepatitis consent/declination on file.
• Two years as PT in acute or rehabilitation setting; home health preferred.

Responsibilities

• Provide one-on-one physical therapy in patients' homes.
• Assess mobility, develop plans, and monitor progress.
• Coordinate care with families and other health professionals.
