Aura Health is seeking a Physical Therapist to join our home health care team, delivering therapy services that enable patients to recover at home. You will assess mobility, set goals, and discharge plans while coordinating with the care team and educating patients and caregivers.

The role includes supervising PT Assistants and Home Health Aides as applicable, and completing required assessments under regulatory guidelines. A valid PT license and CPR are required.

Formación

• Must have a degree in Physical Therapy from an accredited program.
• Current, unrestricted state license to practice PT.
• CPR certification, a valid driver’s license and proof of insurance are required.

Responsabilidades

• Assess mobility, strength and functional status at intake and during care.
• Develop and document plans of care with goals, progress and discharge plans.
• Deliver therapy services per physician orders and ensure compliance.
• Coordinate with the healthcare team and educate patients and caregivers.
• Supervise PT assistants/Home Health Aides as applicable.
• Complete assessments like OASIS where regulation permits and maintain documentation.

• Administer, evaluate, and interpret tests to set functional goals and outcomes.
• Develop and document plans of care, including treatment goals, progress, and discharge plans.
• Deliver therapy services following physician orders and ensure compliance.
• Coordinate with healthcare team members, educate patients and caregivers, and collaborate on discharge planning.
• Maintain accurate documentation to support billing and avoid denials.
• Supervise clinical personnel such as PT Assistants and Home Health Aides as applicable.
• Complete assessments like OASIS where permitted by regulation.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license.
• Valid CPR, driver's license, and proof of insurance.

Preferred Skills

• Experience in home health therapy and OASIS documentation.
