Boost Home Healthcare in Fort Collins, CO is hiring a Home Health Physical Therapist (PT) for full-time, part-time or PRN roles. You will deliver physician-prescribed therapy in patients' homes, perform evaluations, develop plans of care, and coordinate with nurses and physicians to improve mobility and independence.

Qualifications

• Graduate with a Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program.
• PT license in good standing.
• 1 year of clinical PT experience preferred (home health preferred).
• Proficiency with Medicare OASIS documentation and EHR.

Responsibilities

• Deliver physician-prescribed PT in patients' homes.
• Perform initial evaluations and establish care plans.
• Develop maintenance programs and conduct 30-day reassessments.
• Collaborate with nurses, physicians and home care staff for holistic outcomes.
• Provide supervision to PTAs and support personnel as required.

Key Responsibilities (What You’ll Do)

• Deliver Quality In-Home Care: Provide physician-prescribed physical therapy to patients in their residence, helping them optimize mobility and functional independence.
• Patient Assessments & Care Plans: Perform initial evaluations, establish personalized plans of care, develop maintenance programs, and conduct 30-day reassessments.
• Interdisciplinary Collaboration: Participate in patient care planning alongside nurses, physicians, and home care staff to ensure holistic patient outcomes.
• OASIS Documentation: Complete accurate clinical documentation following Medicare guidelines and OASIS protocols.
• Team Supervision: Direct and supervise Physical Therapist Assistants (PTAs) and support personnel as required.

Qualifications & Requirements

• Education: Graduate with a Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited APTA/CAPTE program.
• Licensure: Active Physical Therapist (PT) state license in good standing.
• Experience: Minimum of 1 year of clinical physical therapy experience preferred (Home Health experience highly preferred).
• Documentation: Proficiency with Medicare OASIS documentation and EHR software.
• Core Skills: Strong clinical assessment skills, compassionate patient-first mindset, and excellent communication abilities.
