BAYADA Home Health Care is seeking an experienced Physical Therapist to provide home health visits across Bushkill and surrounding areas. Opportunities include per diem, part-time, or full-time roles, with a focus on delivering rehabilitative care and skilled nursing in clients' homes.

The ideal candidate holds a Pennsylvania PT license, has at least one year of clinical PT experience, and can work independently within a multidisciplinary team.

Formación

• Licensed Physical Therapist in Pennsylvania.
• Minimum of one year of recent PT experience.
• Graduation from an APTA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and collaborative skills.

Responsabilidades

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements related to mobility and gait.
• Implement wound care and protective interventions with evidence-based dressings.
• Develop and implement individualized care plans including therapeutic exercises.
• Document observations, interventions, and evaluations, using EMR.
• Educate clients and family members to promote health and independence.
