Qualifications

• Active Physical Therapist license in Texas (TX).
• Excellent communication skills and a friendly, professional demeanor.
• Ability to adapt care approaches to diverse patients.
• Up-to-date knowledge of evidence-based treatment techniques.
• Experience in home health or rehabilitative settings preferred.

Responsibilities

• Evaluate patients' physical abilities and functional limitations in home health.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress and adjust plans to achieve optimal outcomes.
• Accurately document evaluations and sessions using digital platforms.
• Collaborate with interdisciplinary team members (physicians, nurses, OT).
• Conduct home safety assessments and prevent falls.
