Graham Healthcare Group is seeking a Home Health Physical Therapist to support patients in Gurnee, IL and surrounding areas. This full-time role features a robust benefits package, a 40-hour work week, and an emphasis on team-based care to manage and deliver patient treatment plans.

You will join Residential Home Health and Hospice with a strong track record of employee satisfaction and career growth. A state license, a bachelor's or master’s degree in PT, and a minimum of one year healthcare

Qualifications

• Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college.
• Licensed Physical Therapist issued by the state in which you will be working.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

Responsibilities

• Manage and deliver patient care plans as part of a team.
• Collaborate with clinical educators, preceptors, and supervisors to mentor staff.
• Provide compassionate home health therapy in patients’ homes in Gurnee, IL and surrounding areas.
