Job summary

BAYADA Home Health Care in New Jersey is seeking a Physical Therapist (PT) for full-time home health visits in Atlantic County. New grads welcome. The role offers a sign-on bonus and the chance to build one-on-one patient relationships in clients’ homes.

You will perform comprehensive PT evaluations, develop individualized plans, and educate patients and caregivers while collaborating with a clinical team. Base pay aligns with experience and location.

Qualifications

• Must hold an active NJ PT license.
• Minimum 1 year of recent PT experience.
• Graduation from an APTA/CAAHEA-accredited program.

Responsibilities

• Perform home visits in designated Atlantic County territories.
• Complete comprehensive PT evaluations.
• Assess mobility, strength, balance, gait, transfers, endurance and safety risks.
• Develop individualized treatment plans to improve function.
• Provide therapeutic exercise, gait and balance training.
• Collaborate with nurses and other clinicians as part of a multidisciplinary team.
• Educate patients and caregivers on exercises, safety and disease management.
• Document patient care in the EMR system.

BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT, for a full-time opportunity performing home health visits for our Atlantic County Visits office. This office services adult and geriatric clients on a per visit basis in territories throughout Atlantic County.

As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

What You'll Do

• Perform home visits in designated geographic territories
• Complete comprehensive physical therapy evaluations
• Assess mobility, strength, balance, gait, transfers, endurance, and safety risks
• Develop individualized treatment plans to improve function and independence
• Provide therapeutic exercise, gait training, balance training, and other evidence-based interventions
• Collaborate with nurses and other clinicians as part of a multidisciplinary care team
• Educate patients and caregivers on exercises, safety, and disease management
• Document patient care using a user-friendly electronic medical record system

Qualifications for a PT - Physical Therapist include:

• A current NJPhysical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Additional Information:

Base Pay, depending upon qualifications, status, and subject to negotiation:

$64.00 - $72.00 per point ($99,840 - $112,320 annual earning potential)
