Job summary

PARS Therapy in Navasota, TX seeks a compassionate Physical Therapist to join our home health team delivering patient-centered care in the patient’s home. Candidates must hold a Texas PT license, have a graduate degree in Physical Therapy, and be able to adapt care plans for diverse patients.

Qualifications

• Active Texas Physical Therapy license.
• Graduate degree in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Evidence-based treatment approaches and outcomes-focused care.

Responsibilities

• Evaluate patients in their home and develop personalized treatment plans.
• Provide interventions including exercise, gait training, and manual therapy.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress, adjust plans, and document sessions accurately.
• Collaborate with physicians, nurses, and therapists to optimize care.
