Job summary

PARS Therapy is seeking a compassionate Physical Therapist (PT) to join our home health team in Canyon Lake, TX. This role focuses on helping patients regain movement, reduce pain, and improve their quality of life in the comfort of their own homes.

You will deliver evidence-based, patient-centered care, develop individualized treatment plans, and educate patients and caregivers while collaborating with an interdisciplinary team to promote safety and independence.

Qualifications

• Active Physical Therapist license in Texas (TX).
• Professional liability insurance coverage for TX PT license.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to diverse patients.
• Knowledge of evidence-based treatment techniques and best practices.
• Excellent communication skills and professional demeanor.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in home health.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapeutic interventions: exercise, gait training, manual therapy, balance retraining.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress and adjust plans to achieve optimal outcomes.
• Document evaluations, sessions, and progress notes using digital platforms.
• Collaborate with physicians, nurses, and therapists.
• Conduct home safety assessments to prevent falls or re-injury.
• Deliver compassionate care while promoting independence.
