Job summary

PARS Therapy in Cypress, TX is seeking a compassionate Physical Therapist (PT) to join our home health team. You will evaluate patients, develop and implement personalized treatment plans, and provide hands-on interventions in patients' homes to improve mobility and reduce pain.

The ideal candidate holds an active Texas PT license, graduate degree in Physical Therapy, and strong communication skills. Experience in home health and commitment to evidence-based practice are preferred; we offer

Qualifications

• Active Physical Therapist license in Texas.
• Experience in home health or rehabilitative settings is preferred.
• Strong ability to adapt care approaches to variety of patient needs.
• Up-to-date knowledge of evidence-based treatment techniques and best practices.
• Excellent communication skills when educating and interacting with patients.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies.
• Monitor patient progress and adjust plans to achieve optimal outcomes.
• Document evaluations, treatment sessions, and progress notes using digital platforms.
• Collaborate with physicians, nurses, and occupational therapists.
• Conduct home safety assessments and make recommendations to prevent falls or re-injury.
• Deliver compassionate care while empowering patients to achieve greater independence.
