Job summary

Aveanna Healthcare is seeking a Visiting Physical Therapist for our VNA Certified division to evaluate, coordinate, and administer quality physical therapy services in patients' homes. The role supports a range of patients and ages with chronic conditions and therapy needs.

Pay is $65 per hour/visit with flexible full-time or part-time scheduling and weekend options. Clinicians travel with a degree of autonomy and access to local support, documentation tablets, and ACA eligibility after the

Responsibilities

• Deliver physical therapy services according to physician orders.
• Follow policies, procedures, and state regulations for patient care.
• Provide written evaluations defining PT needs, goals, and progress.
• Coordinate services with the health care team and family.
• Supervise other personnel as applicable.
• Complete OASIS assessments where state regulations allow.

Job description

Our Visiting Physical Therapist for our VNA Certified division is responsible and accountable for evaluation, coordination, and administration of quality physical therapy services.

At Aveanna, our services include Skilled Nursing, Physical Therapy, Occupational Therapy, and Home Health Aide support delivered in patients’ homes. We serve diverse populations across all ages, including patients with chronic conditions, behavioral health needs, skilled nursing requirements, and therapy needs.

Pay

$65 per hour/visit based on the pay point model

What You’ll Do

• Delivery of physical therapy services to comply with prescribed physician orders.


• Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws.


• Provide a written evaluation defining physical therapy needs, goals, progress and expected outcomes in conjunction with referring physician’s Plan of Treatment.


• Participates in coordination of services (including discharge planning) with health care team, physicians, parent/guardians, nurses, etc.


• Maintain effective communication between staff, healthcare team members and family.


• Serve as a resource consultant for clinical staff.


• Supervises other personnel (PTA, HHA) as applicable.


• Completes OASIS assessments where allowed by state professional practice and regulation.

Qualifications

• A certification in Physical Therapy in the state of application


• Current and unrestricted license in the state of practice


• Current CPR certification (with hands-on component) or able to obtain one


• Current TB test or able to obtain one


• Must have a drivers license and reliable transportation
