Job summary

Aveanna Healthcare is seeking a Visiting Occupational Therapist to evaluate, coordinate, and administer quality OT services in patients' homes through our VNA Certified division. The role includes flexible full-time/part-time scheduling and per-visit pay.

You will deliver OT services, perform evaluations, plan care with physicians, and ensure safe, effective treatment while coordinating with the health care team.

Qualifications

• OT certification in the state of application is required.
• Current unrestricted license in the state of practice.
• CPR certification with hands-on component (or ability to obtain).
• Current TB test (or ability to obtain).
• Driver’s license and reliable transportation.

Responsibilities

• Provide occupational therapy services as prescribed by the physician.
• Write evaluation defining OT needs, goals and outcomes with Plan of Treatment.
• Draft Plan of Care from nursing staff for continuing care.
• Coordinate services and discharge planning with care team and family.
• Administer care to ensure patient safety and monitor condition.
• Maintain effective communication with staff and family and document progress.
