Job summary

Premier Home Health Providers Inc. is seeking a dedicated Physical Therapist to deliver in-home PT services in California. The role is per diem with visits at $160–$180 each, focusing on improving mobility, independence, and safety in the patient’s home.

Responsibilities include evaluating functional status, developing treatment plans, educating patients and families, and coordinating with the care team. A CA PT license and CPR certification are required, with at least 2 years of PT experience;

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills and the ability to connect with patients and families.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.

Responsibilities

• Evaluate patients’ functional status, mobility, strength, gait, and ROM to plan PT care.
• Provide PT services per physician plan and California PT Act guidelines.
• Educate patients and families on exercises, equipment use, and goals.
• Collaborate with physicians and care team, report changes, participate in conferences.
• Supervise PT Assistants and oversee HHA care when PT is sole skilled service.
• Maintain documentation including progress notes and discharge planning.
• Identify needs for additional home health services and coordinate referrals.
• Perform other duties as required for patient services.
