Job summary

Premier Home Health Providers Inc. seeks a per diem Physical Therapist to support in-home patient care across California. You will assess mobility, develop plans, and deliver skilled PT services under a physician’s plan of care, ensuring compliant, high-quality care in patients’ homes.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services per physician’s plan of care and California PT Practice Act.
• Educate patients and caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team; report changes in condition and participate in case conferences.
• Supervise PT Assistants and oversee Home Health Aide care when PT is sole skilled service.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.
